Join us as Jean Osuch and Rich Miller-Murphy discuss how Five Wishes can help you and your family as well as who should use it and why. The Five Wishes is the first living will (also called an advance directive) that helps facilitate personal, emotional, and spiritual needs as well as an individual’s medical wishes, allowing an individual to say exactly how they wish to be treated if they become seriously ill. This program will help guide a person (and their family) by providing examples.

All community members who register for the program will receive a complimentary workbook and instructions for use. There is no charge for this community education program.

Jean Osuch attended Excelsior University College of Nursing and Allied Health Sciences and Liberty University where she received her Bachelor of Science in Nursing (BSN). Jean has extensive clinical experience for over twenty years with both Regional Hospice & Homecare of Western CT as a nurse case manager, and has directed the hospice program at Visiting Nurse and Hospice of Litchfield County. She is also a Certified Hospice and Palliative Care nurse (CHPN).

Rich Miller-Murphy attended Fairfield University where he received his Bachelor of Science in Biology and Chemistry, then completed his master’s from the University of New Haven. Later, after completing his Master of Arts in Chaplaincy (MAC) from Hartford Seminary, he left the medical equipment industry to focus on end-of-life options. Recently he worked with Senator Harding to pass SB431 to explore end-of-life options through a new pilot program educating the public in Litchfield County’s 26 towns.
